Engineering ClaimsI. GENERAL PROCEDURES 1. Immediately notify the concerned agent/broker or us about the loss by telephone, fax or email, as well as through a formal letter. Give an indication as to the nature and extent of loss or damage. We shall not in any case be liable for loss, damage, or liability of which we have received no notice within 14 days of its occurrence. 2. Take all steps within your power to minimize the extent of the loss or damage. 3. Preserve the parts affected and make them available for inspection by our representative or surveyor. 4. Furnish all such information and documentary evidence as we may require. 5. Inform the police authorities in case of loss or damage due to theft, burglary, or sabotage. 6. Within sixty (60) days from the date of loss, submit a proof of loss duly signed and sworn to, stating: 1. The knowledge and belief of the time/origin of the loss 2. Amount of loss encumbrances 3. All other contracts of insurance 4. All other information related to the insured properties 7. If the claim is substantial and/or complex, we will appoint an independent adjuster/surveyor to handle the investigation of the claim. The adjuster/surveyor will shortly get in touch with you for inspection of the insured property and verification of the related incident. 8. Make yourself and/or your representatives available for the inspection of the incident. 9. Submit either to us or to the appointed independent loss adjuster all documents necessary to facilitate processing and settlement of your claim. II. LIST OF DOCUMENTS REQUIRED BY TYPE OF POLICY 1. General Requirements 1. Sworn statement in proof of loss 2. Non-waiver agreement 3. Detailed incident report 4. Photographs 2. For CAR and EAR claims 1. Full copy of the contract agreement 2. Breakdown of the contract value 3. Detailed rehabilitation/repair cost estimate for the damaged properties 4. PERT/CPM and/or bar chart as necessary 1. Accomplishment report as to the time of the loss (as necessary) 2. Other technical plans as necessary (construction plans and/or drawings, as built plans and/or drawings, structural analysis and design, etc) 3. For EEI, Machinery Breakdown, & Boiler/Pressure Vessel Claims 1. Inventory of damaged and/or affected machinery and equipment showing the new replacement values, dates acquired and models/serial numbers with comments on the extent of the damage 2. Purchase invoices, delivery receipts, suppliers' certificates, importation documents and all other records pertaining to the damaged equipment 3. Quotations from qualified suppliers on repair/reinstatement of the affected properties 4. Witness/es' statements (as necessary) 5. Copy of the engineer's technical report with comments as to the probable cause of loss, extent of damage, and emergency measures taken 4. For Theft and Burglary Claims 1. Photographic evidence of the point of entry and/or exit 2. Police report 3. Inventory of stolen and/or missing properties 4. Documentary evidence as proof of loss, e.g. invoices, etc. 5. For Third Party Liability Claims 1. Bodily injury and/or death 1. Original copies of medical receipts and other related expenses 2. Doctors' certification regarding the extent of injury 3. Copy of formal compliant/subpoena if the third party has taken legal action against the insured 4. Written statements of the witnesses 2. Property damage 1. Repair cost estimate for the damaged properties 2. Quotations for the repair Pioneer Insurance and/or our representatives reserve the right to request for additional documents/information as may be necessary in the course of the evaluation of the claim/s. |
